Delayed Contact Precautions and Roommate Exposure During C. difficile Infection

Summary

The facility failed to establish and maintain an infection prevention and control program for two residents who were involved in a c. difficile infection event. One resident had a history that included acute respiratory failure, COPD, legal blindness, anxiety, and type 2 diabetes, and was identified as being at risk for infection or recurrent/chronic infection. She developed nausea, vomiting, and diarrhea, and a stool specimen collected for c. difficile testing later returned detected for toxigenic c. difficile. A nurse documented the positive result and noted that the NP was notified and that orders were pending, and another note documented a new order for vancomycin and contact precautions. The record showed that the contact isolation order was not entered into the electronic medical record until 6/14/26, even though the lab result was reviewed on 6/12/26 and the positive result had been reported to the facility on 6/11/26 at 9:47 p.m. The resident remained in a semiprivate room during this period. The facility’s infection prevention and control policy identified c. difficile as an infection requiring contact precautions and stated that a private room should be used when available. A second resident, who had diagnoses including metabolic encephalopathy, dementia, and muscle weakness and was dependent on staff for all ADL care except eating, was the roommate of the first resident until 6/14/26. Nursing documentation stated that the roommate was moved to another room due to isolation precautions. Staff interviews indicated that the roommate was moved before precautions were entered, and one CNA stated that contact precautions and the PPE cart were set up the day before the first resident was sent to the hospital. The DON and other staff gave differing accounts about when precautions were initiated and whether PPE was used before the order appeared in the system.
