PASRR Specialized Services Not Properly Documented or Submitted
Summary
The facility failed to incorporate the recommendations from the PASRR Level II determination and PASRR evaluation report into Resident #8’s assessment, care planning, and transitions of care. Resident #8 was a female with diagnoses including spastic quadriplegic cerebral palsy, anxiety disorder, depressive disorder, severe intellectual disability, speech and language development delay due to hearing loss, ADHD, contracture, and neuromuscular scoliosis. Her quarterly MDS showed she was severely impaired in daily decision making, and her PASRR Level 1 screening was positive for a known or suspected intellectual disability. Record review showed the PASRR evaluation identified the resident as having an intellectual disability and requiring specialized services. The PCSP documented that the IDT meeting was held with the resident, PASRR Habilitation Coordinator, Local Authority for IDD, social worker, MDS Coordinator RN, QIDP, legal guardian, and nursing facility representatives. The NFSS identified and confirmed included durable medical equipment, and the comments summary stated the resident was not on therapy services at that time, while the habilitation coordinator and guardian recommended specialized PT and OT assessments. The Local Authority for IDD confirmed the selected specialized services, and PT and OT evaluation and treatment began after the meeting. The facility failed to submit a complete and accurate request for nursing facility specialized services in the LTC Online Portal within 20 business days. The portal was later reopened to allow resubmission reflecting the IDT meeting, but the submission remained incomplete and was not closed. The facility policy stated the request for specialized services was to be initiated within 20 business days of the IDT meeting and that the MDS Coordinator or designee would document the IDT minutes and all specialized services into the online portal within 3 business days after the IDT meeting and annually thereafter. Interviews with the MDS Coordinator, Administrator, and Mental Health Provider staff reflected confusion about the submission status, scheduling, and responsibility for the PCSP entry, and the record showed no completed PCSP submission through the portal.
