PASRR Specialized Services Request Not Submitted After IDT Meeting
Summary
The facility failed to incorporate the recommendations from the PASRR Level II determination and PASRR evaluation report into the resident’s assessment, care plan, and transitions of care for one resident reviewed for PASRR services. The resident was a female with diagnoses including seizure disorder, respiratory failure, cognitive communication deficit, renal failure, hypertension, and heart failure. Her comprehensive care plan identified her as PASRR positive for intellectual disability and included interventions for an IDT meeting, notification of the local authority for significant changes, notification of the physician and responsible party, completion of a PASRR evaluation by the local authority, and provision of specialized services as determined by the IDT meeting. Review of the HHSC PASRR spreadsheet showed the resident was listed for CMWC and DME services, and facility email records showed the ADM and previous DON were notified by the HHSC PASRR unit that an IDT meeting had been held and entered into the LTC Online Portal and that nursing facility specialized services had been recommended and agreed upon. The email stated the facility was out of compliance because the HHSC PASRR unit had not received a request for specialized services for the resident. Additional email communication between the facility and the LIDDA showed the facility asked whether the resident had requested a CMWC after the MDSC who opened the form was no longer with the company, and the LIDDA confirmed the resident did have a CMWC added to her plan. The facility then stated that the previous MDSC had not communicated the need for a CMWC and noted that business day 20 had already passed. The LIDDA responded that the facility had 20 days after the IDT meeting to submit services and that a meeting would need to be planned to restart the clock. During interviews, the MDSC stated she did not attend the resident’s IDT meeting because the previous MDSC was present and no longer worked at the facility. The ADM stated the MDSC who attended the meeting had gone on personal leave and never returned, and that the second MDSC or DON was responsible for ensuring deadlines were followed when personnel changes occurred. The facility’s PASRR flow chart stated that if the NF could serve the individual, the NF would conduct the IDT within 14 calendar days and then request specialized services within 20 business days of the IDT.
