Summary

Provide and implement an infection prevention and control program was deficient when the facility failed to establish and maintain infection prevention and control practices for 1 of 3 residents reviewed for infection control. Resident #1 was a [AGE]-year-old female with diagnoses including cerebral infarction, muscle wasting and atrophy, and Type 2 diabetes. Her care plan reflected that she required assistance from 1 staff member to turn and reposition in bed as necessary, and her physician orders included daily and every-shift treatment for a pressure injury on the left gluteus using Dakins solution, Santyl, gentamicin ointment, and a bordered foam dressing. During an observation of wound care, the nurse and CNA washed hands before treatment and after treatment was completed, donned PPE properly, and sanitized hands between glove changes. The wound dressing was labeled and had no drainage or odor, and the LVN performed the wound care according to the physician order. However, the LVN lathered her hands for 9 seconds during the procedure. In an interview, the LVN stated she should have lathered her hands for at least 20 seconds and said she thought she had done so. The DON later stated proper handwashing was to lather hands for at least 20 seconds and that failing to do so would put the resident at risk for infections. The facility policy identified hand hygiene as the primary means to prevent the spread of infections.
