Summary

The facility failed to ensure drugs and biologicals were stored and labeled in accordance with currently accepted professional principles for 1 wound care cart out of 7 medication carts. During an observation on 07/29/2026 at 4:36 PM, the wound care medication cart was found left unlocked on the 200 hallway while unattended by LVN B. When the surveyor pointed out that the cart was unlocked, LVN B secured it by locking it. LVN B stated she was responsible for the cart and that she was expected to lock it when walking away from it, but she had left it unlocked because she forgot. During an interview later that evening, the DON stated that staff were expected to lock medication carts whenever they walked away from them and that all staff were responsible for locking and monitoring the medication carts. The DON also stated that the negative outcome of leaving the cart unlocked was that a resident or visitor could grab medication from the cart and be harmed. Facility policy Medication Cart Use & Storage, revised January 2023, stated that the medication cart and its storage bins should be kept closed, secured, and/or in the line of sight when not in use.
