Late PASRR NFSS Submissions for Two Residents
Summary
The facility failed to incorporate recommendations from PASRR evaluation reports into the resident assessment, care planning, and transition of care for two residents reviewed for PASRR services. The deficiency involved failure to submit a complete and accurate request for nursing facility specialized services in the LTC online portal within 20 business days after the IDT meetings for both residents. The report stated this could place residents who were PASRR positive at risk of not receiving PASRR services, which may enhance quality of life, and could lead to a decline in health. Resident #1 was a male with diagnoses including cerebral palsy, cognitive communication deficit, degenerative disease of the nervous system, muscle weakness, and unspecified lack of coordination. His annual MDS showed a BIMS score of 09, indicating moderate cognitive impairment. His PCSP documented an IDT meeting with the LIDDA Habilitation Coordinator, Social Worker, MDS nurse, Director of Rehabilitation, and the resident, and identified NFSS including a customized manual wheelchair, durable medical equipment, specialized OT, PT, and ST assessments and services, as well as day habilitation and habilitation coordination services. The facility’s LTC portal showed the NFSS was not submitted until well after the IDT meeting. Resident #2 was a male with diagnoses including cerebral palsy, cognitive communication deficit, reduced mobility, lack of coordination, paraplegia, and difficulty walking. His annual MDS showed a BIMS score of 14, indicating slight cognitive impairment. His PCSP documented a quarterly IDT meeting with the PASRR Habilitation Coordinator, Social Worker, Director of Rehabilitation, MDS nurse, Dietary Manager, and the resident, and identified NFSS including a customized manual wheelchair, durable medical equipment, specialized OT, PT, and ST assessments and services, plus habilitation coordination services. The facility’s LTC portal showed the NFSS was also submitted late. During interviews, the MDS nurse stated she was new, had not completed an NFSS since starting, was not familiar with the process or timeframes, and did not know why the submissions were not timely. The DOR stated she had never completed an NFSS and was not familiar with the process, and the Administrator stated the DOR was responsible for submission and the MDS nurse for monitoring acceptance.
