Water Dripping From Ceiling Fixture and AC Ducts

Brownsville Nursing And Rehabilitation CenterBrownsville, Texas Survey Completed on 07-10-2026

Summary

The facility failed to provide a safe, clean, and sanitary environment for 2 of 10 rooms, including rooms [ROOM NUMBER] and [ROOM NUMBER], because a lighting fixture and AC ducts were not maintained free of dripping water and water damage. During observation on 07/10/26 at 3:30 p.m. and again at 7:15 p.m., surveyors saw a lighting fixture in the ceiling above the beds dripping water. During an interview and observation on 07/10/26 at 7:45 p.m., the Maintenance Director stated the leak and lighting fixture had been dripping condensation for about 2 weeks and said he had tried to fix it, but was not aware the water continued to drip. He stated that if the holes were filled in, condensation would continue to collect in the electrical conduit box and have nowhere to drain. During an interview on 07/10/26 at 8:00 p.m., the ADM stated one of the AC ducts had a leak, that the company had come to the facility and ordered parts to fix the AC conduct, and that the duct made condensation and caused water to drip. The ADM also stated the facility did not have a policy on environment.
