Crushed ER Potassium Administered Against Label Instructions
Summary
The facility failed to ensure that two residents were free from significant medication errors when MA B crushed Potassium Chloride ER tablets and administered them by mouth, despite the blister packets stating, "Swallow Whole Do not Chew Or Crush." Resident #1 was an older female admitted with diagnoses including low potassium, hypertension, Alzheimer's disease with early onset, anorexia, a history of transient ischemic attack, cerebral infarction, and a personal history of breast cancer. Her quarterly MDS showed a BIMS score of 4, indicating severe cognitive impairment, and she required extensive assistance with ADLs and limited assistance with eating. Her care plan and physician's order directed that Potassium Chloride ER 20 mEq be given by mouth once daily, and the MAR reflected the same order. During observation of medication administration, MA B removed the Potassium Chloride ER 20 mEq from the blister packet, punched it into a medication cup, added other medications including amlodipine and calcium, crushed all of the medications, mixed them with yogurt, and administered them to Resident #1 by mouth. The blister packet was observed to contain the instruction to swallow whole and not chew or crush. The medication was scheduled for 9:00 AM, and the administration was observed at 10:15 AM. Resident #2 was an older female admitted with diagnoses including fatigue, gangrene, aphasia, chest pain, shortness of breath, low potassium, and hypertension. Her quarterly MDS showed severe cognitive impairment, and she required extensive assistance with ADLs and limited assistance with eating. Her care plan directed that Potassium Chloride be given as ordered, and her physician's order and MAR reflected Potassium Chloride ER 10 mEq by mouth daily. During observation, MA B similarly removed the tablet from the blister packet, crushed it with Eliquis and Metoprolol, and mixed it with yogurt. After surveyor intervention, MA B checked the packet instructions, acknowledged that the packet said swallow whole and do not chew or crush, stated that Resident #2 could not swallow the potassium whole, and continued to administer it by mouth. MA B later stated she had not checked the blister packet instructions and that crushing the potassium would not work as it should in the residents' blood. The DON stated she was not aware of the issue and that the doctor would be notified for stat potassium blood work.
