Unclean shower room and unlined trash cans
Summary
The facility failed to provide a safe, functional, sanitary, and comfortable environment in 1 of 4 hall shower rooms and in 2 of 8 resident rooms reviewed for environment. During observation, the 300 hall shower room was found with a molded/rusted shower seat, peeling shower seat material, used gloves, washcloths, plastic bottles, pillows, a broken bedside table frame, an old shower head, used tissue paper, and plastic wrappings scattered on the floor. The trash can in the shower room contained trash but had no liner, and the toilet seat had a stain on it. Personal belongings and drinks were also observed on a bedside table and chair in the shower room. Resident #1 was a female admitted to the facility with a diagnosis of need for assistance with personal care. Her quarterly MDS assessment showed a BIMS score of 07, indicating severe cognitive impairment, and she required substantial to maximal assistance with toileting hygiene, showering, lower body dressing, and putting on or taking off footwear. During observation, the trash can in her room had no liner. Resident #4 was a male admitted with muscle weakness and muscle wasting and atrophy. His quarterly MDS assessment showed two unstageable pressure injuries and one stage 4 pressure ulcer present on admission, and he was coded for risk of developing pressure ulcers/injuries. During observation, the trash can in his room also had no liner. During interviews, the Maintenance/housekeeping director stated housekeeping cleans shower rooms, CNAs clean after showers, and the shower room on 300 hall had not been paid attention to. He stated the old shower head had been changed two days earlier and the maintenance helper got distracted and forgot about it. He also stated there was no log to track shower room cleaning, though one could be made, and said not deep cleaning the shower room could cause cross-contamination and spread disease. The CNA stated shower rooms could be better cleaned and that no drinks or personal items should be stored there because of contamination. The DON and Administrator stated the shower room should always be clean and clear of debris, no personal items should be stored in the shower room, and trash cans should always be lined.
