Unsafe Resident Transfers Without Required Lift Equipment

Summary

The facility failed to ensure a resident’s environment remained free of accident hazards and failed to provide adequate supervision and assistance devices during transfers for a resident with left-sided hemiplegia and hemiparesis following a stroke, COPD, heart failure, peripheral vascular disease, prior left humerus fractures, osteoarthritis, weakness, lack of coordination, and abnormal gait and mobility. The resident’s MDS indicated she was cognitively intact, used a wheelchair, and was dependent on staff for most ADLs, including tub/shower transfers and chair/bed transfers. Her care plan required total assist of two staff and a mechanical lift for transfers, with 2-3 person total assist only if equipment was unavailable. Video footage showed CNA A attempting to transfer the resident from a shower chair to her wheelchair and later from her wheelchair to her bed by lifting her under the arms without a gait belt or other assistive device. During the bed transfer, the resident began to slip through CNA A’s hands, and CNA B and CNA C then assisted in pulling her back into the wheelchair. The resident was then transferred to bed by staff lifting her under the arms and dragging/turning her without a gait belt or lift pad. The resident was heard asking to be put on the bed and later said she was hurting and asked for pills. Staff were heard discussing the shower chair and the mechanical lift, including comments that the chair needed to be replaced and that someone could get stuck midair. Interviews confirmed staff knew the resident required a mechanical lift or other safe transfer method. CNA B stated the resident almost slipped during the transfer and that staff should not have lifted her under the arms. CNA C stated the resident needed a mechanical lift or at least 3-4 people and said the resident could have fallen and been injured. CNA A gave inconsistent accounts but acknowledged training on gait belt use. ADON E stated she was told staff almost dropped the resident, but she did not document an incident report because the resident did not fall. She also stated the family later showed her the video of the unsafe transfer. The DON, ADM, and Regional Nurse reviewed the video and stated the transfer was not safe and that staff should have used a sheet or the mechanical lift pad rather than lifting the resident under her arms.
