Failure to Follow Mechanical Lift Transfer Care Plan

Summary

The facility failed to develop and implement a comprehensive care plan for a resident with significant mobility limitations and multiple diagnoses, including left-sided hemiplegia and hemiparesis following a cerebral infarction, COPD, heart failure, peripheral vascular disease, prior left humerus fractures, osteoarthritis, pain, lack of coordination, weakness, and abnormal gait and mobility. The resident’s MDS indicated she was cognitively intact, used a wheelchair, and was dependent on staff for most ADLs, including transfers. Her care plan directed that she required total assist of two staff and a mechanical lift for transfers, with 2-3 person total assist only if equipment was unavailable. Video footage showed CNA A attempting to transfer the resident from her wheelchair by placing his arms under both of her arms and lifting her without a mechanical lift or gait belt. As he tried to turn her toward the bed, she began to slip through his hands and her lower back was at the level of the wheelchair seat. CNA B and CNA C then assisted in pulling her back into the wheelchair. The resident was wearing only a hospital gown with the back open, and no gait belt or other assistive device was being used. The video then showed CNA C and CNA A lifting and dragging the resident toward the bed while CNA B pushed her bottom toward the bed, after which they lifted her legs onto the bed. The resident stated, "put me on the bed" and later said, "I'm hurting" and asked for pills. Staff then rolled her side to side, placed a brief on her, and repositioned her in bed. Interviews with staff showed they knew the resident was a mechanical lift transfer and that lifting her under the arms without proper equipment could cause injury, but they described the lift as not working because of battery issues and proceeded with manual transfers anyway. The facility’s own policy stated that comprehensive care plans must include measurable objectives and timetables and be implemented to meet the resident’s physical, psychosocial, and functional needs.
