Delayed Custom Wheelchair Evaluations for Residents With Limited ROM

Summary

The facility failed to ensure that three residents with hemiplegia, hemiparesis, and limited range of motion received timely referrals for customized wheelchair evaluations. Resident #1 had diagnoses including left-sided hemiplegia/hemiparesis, COPD, and muscle weakness, and his MDS showed he was dependent on staff for walking, standing, and mobility with a manual wheelchair, with functional limitations in ROM to the left upper and lower extremities. His order summary did not address the referral for a customized wheelchair, and emails from rehab staff showed the evaluation had been requested but was not scheduled. During observation, he was seen propelling himself backward in a manual wheelchair using his right foot and reported bumping his right foot and concern about skin injury. Resident #2 had diagnoses including left-sided hemiplegia/hemiparesis, COPD, and muscle wasting and atrophy. His MDS showed he was dependent for all ADLs except eating, had functional limitations in ROM to the left side, and used a manual wheelchair. His order summary indicated he needed evaluation by an outside clinic for a custom standard wheelchair to improve posture, comfort, and provide a specialty cushion. Rehab documentation showed the request had been made, but the outside clinic reported he was not scheduled. During observation, he was slumped and leaning forward in the wheelchair and had to be repositioned upright by staff. He stated he needed another wheelchair because the curvature of his spine caused him to lean onto his left arm with pressure. Resident #3 had diagnoses including left-sided hemiplegia/hemiparesis and muscle wasting and atrophy. His MDS showed he was dependent for all ADLs except eating, had functional limitations in ROM to the left side, and used a manual wheelchair. His order summary indicated he was to be evaluated by an outside clinic for a custom wheelchair, and rehab documentation showed the request had been entered, but no appointment had been scheduled. A family member stated it had been at least a month since the consult was made and she had not heard anything about the wheelchair evaluation. The Rehab Director, Director of Transportation, ADON, and DON all discussed the lack of scheduled consults, and the Director of Transportation stated she could not locate any scheduled consults for the three residents.
