Standing Water and Unsanitary Conditions in Shower and Utility Areas

Summary

The facility failed to maintain a safe, sanitary, orderly, and comfortable environment in 2 areas on the 200 Hall: the Shower Room and the Soiled Utility Room. During observation, the Shower Room bed liner underneath the shower bed had 4 puddles of standing water that appeared light brown, and the liner itself appeared white with areas that were brown and black. The Shower Room was observed more than once with the same standing water present. The Soiled Utility Room sink on the 200 Hall was also observed filled with standing water that appeared light brown. A glove, a clear plastic cup, and two other unknown items were floating in the sink, and the sink appeared white with brown dirt. The sink was observed again later with the same standing water and items still present. During the observation period, a confidential resident was seen throwing an empty soda can box into the trash can in the Utility Room. Interview information showed that a resident's RP had previously seen the same puddles of brown water in the Shower Room bed liner and had raised concerns to staff. A resident also stated the Shower Room had dirty, brown puddles with an unknown substance and that the Utility Room sink had standing water for an unknown period. CNA A stated standing water should not be present in either area and described the Utility Room sink as a place used to rinse soiled items before they were sent to laundry. The DON and Maintenance Staff C both acknowledged the standing water in the Utility Room sink, and Maintenance Staff C stated the Shower Room liner had been removed by staff before he could inspect it. Record review showed no grievance entries related to either area.
