Dietary Manager Touched Mouth While Preparing Meal Trays

Focused Care At HuntsvilleHuntsville, Texas Survey Completed on 07-15-2026

Summary

The facility failed to store, prepare, distribute, and serve food under sanitary conditions in 1 of 1 kitchen reviewed for food and nutrition services. During an observation on 07/13/2026 at 11:40 AM, the dietary manager was preparing meal trays for lunch and was observed putting the fingers of her right hand to her mouth and touching her tongue before picking up a resident tray card and placing it on the tray. She then placed the plate warmer, plate, utensils, condiments, and drinks on the tray before passing the tray to the cook. She repeated the same action while preparing the next tray, again touching her mouth before handling the meal card and tray items. When approached, the dietary manager stated the papers were sticking together and she was trying to keep everything moving. She stopped preparing trays, washed her hands, and instructed the cook to wash her hands also. During interview, she stated she did not normally prepare trays during meal service and was doing so because the scheduled dietary aide did not report to work. She acknowledged that touching her mouth and then handling tray items should not have happened and could increase the risk of illness or infection. The Administrator stated staff should wash their hands after touching their face and before touching plates, utensils, or trays, and the facility policy titled Hand Hygiene stated hand hygiene should be performed before touching the mouth, nose, or eyes and after contact with mucous membranes such as the mouth or nose.
