Summary

The facility failed to store drugs and biologicals in locked compartments and permitted medications to be kept inside resident rooms for five residents reviewed for medication storage. During observations, a hemorrhoid ointment was found on top of one resident’s side table, a laxative solution was found inside another resident’s opened drawer, and a nasal spray was found on top of a third resident’s side table. None of these residents had orders for those medications, and none had an assessment showing they were competent to self-administer medications. A resident with a psychotic disorder and a BIMS score of 15 had a hemorrhoid ointment in plain view on the side table while not in the room. Another resident with cognitive impairment and depression had a bottle of laxative solution inside an opened drawer within reach. A third resident with acute upper respiratory infection and a BIMS score of 08 had a nasal spray on the side table and stated it was hers and that she used it sometimes. An LVN stated medications were not supposed to be inside resident rooms because staff should administer them and because residents could overuse, underuse, or use medications inappropriately. The facility also failed to keep wound care items secured for another resident with depression and delusion who had orders for wound care to the scalp and right leg. Containers of wound cleanser and collagen powder were observed on top of the resident’s side table in a tray with mouthwash. A fifth resident with cellulitis had a cup of topical antibiotic left on top of the overbed table by an LPN. The facility policy stated medications and biologicals are to be stored safely and securely, accessible only to authorized personnel, and medication rooms, carts, and supplies are to be locked or attended by persons with authorized access.
