Summary

The facility failed to keep resident areas free of hazards when an uncovered razor was found at a resident’s sink in his room. The resident was cognitively intact with a BIMS score of 14 and required substantial assistance for personal hygiene. His care plan included assistance with personal hygiene, including shaving. During observation, he was in bed awake and said he did not know who left the razor in his room, though staff used it to shave the hairs on his chin. The facility also failed to keep a liquid disinfectant out of a resident’s room. The resident was cognitively intact with a BIMS score of 15 and had diagnoses of depression and delusion. During observation, a container of liquid disinfectant was on top of the resident’s side table in a tray with his mouthwash. The resident said the solution was used to clean his room and had been left by a staff member, but he could not remember who left it there. An LVN later observed the disinfecting solution in the room and removed it, stating it should not be left where residents could access it. The facility further failed to keep sharps and germicidal wipes unattended and within resident reach. A wound care nurse left scissors on top of a resident’s overbed table and stepped out of the room to assist another staff member, leaving the scissors within reach of the resident. In addition, a container of germicidal wipes was observed left open on top of a cart in the hallway while residents passed by, and another container of germicidal wipes was left at the edge of the nurses’ station unattended and within reach of residents. Staff and administration stated the wipes, disinfecting solution, razor, and scissors should not be left unattended or within resident reach.
