Failure to Complete Significant Change MDS After Pressure Ulcers and Behavior Changes

Deerings Nursing And RehabilitationOdessa, Texas Survey Completed on 05-21-2026

Summary

The facility failed to complete a comprehensive assessment within 14 days after a significant change in condition for one resident. Resident #7 was a female admitted on 11/8/2024 with diagnoses including chronic venous hypertension with bilateral lower extremity complications, peripheral vascular disease, and unspecified protein calorie malnutrition. Her record also showed diagnoses of delirium and delusions added on 4/1/2026, and a stage IV pressure ulcer of the right elbow added on 4/8/2026, along with a stage III pressure ulcer diagnosis added on 12/2/2025. Review of the resident’s MDS assessments showed a quarterly assessment with a BIMS score of 6 and no coded behaviors or pressure ulcers, followed by a later quarterly assessment with a BIMS score of 7, behaviors occurring 1 to 3 days, rejection of care and wandering occurring 1 to 3 days, and a pressure ulcer coded as present. The later assessment also documented one unstageable pressure ulcer/injury not present on admission or reentry. Wound care documentation dated 5/19/2026 showed a stage IV pressure wound to the right elbow present for greater than 118 days, a stage III wound to the left heel present for greater than 17 days, and a stage III wound to the right posterior thigh that was resolved that same day. The order summary dated 5/21/2026 listed treatment orders for the stage IV right elbow pressure ulcer, the stage III left heel pressure wound, and the stage III right posterior thigh wound. During interviews, the DON stated the MDS Coordinator was responsible for completing the Significant Change in Status MDS after the interdisciplinary team identifies the significant change, and stated she did not realize pressure ulcers or a change in behavior were causes for a Significant Change in Status MDS. The MDS Coordinator stated she was not aware that a Significant Change MDS needed to be completed and said the failure could cause the resident to miss care areas such as wound care or other care areas not being identified and/or a comprehensive care plan not being completed.

Failure to Complete Significant Change MDS Assessment
D
F0637 F637: Assess the resident when there is a significant change in condition
Short Summary

Failure to Complete Significant Change MDS Assessment: A resident with dementia, progressive neurological conditions, and seizure disorder had an admission MDS that coded no mobility device use and independence with ADLs, but later was observed ambulating with a walker and gait belt. Quarterly MDSs still did not code walker use and showed increased ADL assistance needs, yet no comprehensive or significant change MDS had been completed. MDS coordinators said they were unaware of the walker use, believed the resident likely should not have been coded as independent, and confirmed the significant change MDS was missed.

Short Summary

The facility failed to complete an MDS SCSA within the required timeframe after a resident elected hospice. One resident with dementia, osteoporosis, AFib, HTN, and depression had a right fibular fracture after a foot/ankle injury, and another resident with dementia, TIA history, stroke history, hypothyroidism, and bilateral above-the-knee amputations had a left elbow fracture.
