PASRR Screening Not Updated After Schizophrenia Diagnosis
Summary
The facility failed to ensure a resident was screened for mental illness and that the State mental health authority was coordinated with after the resident received a diagnosis of Schizophrenia. The resident’s record showed diagnoses of Anxiety Disorder, Depression, and Schizophrenia, along with impaired cognition, disorganized thinking, and significant assistance needs for activities of daily living. The care plan reflected cognitive impairment and a history of refusing care related to schizophrenia, and the resident was receiving antidepressant medication for depression. The resident’s PASRR Level 1 screening dated 10/22/24 indicated no evidence of mental illness at that time. After the psychiatrist diagnosed Schizophrenia, the clinical chart contained no PASRR Level 1 screening or Mental Illness/Dementia Resident Review (Form 1012) completed for the new diagnosis. During interviews, the MDS Coordinator stated the resident had a negative PASRR screening in 2024 because there was no evidence of mental illness at that time, and the Regional MDS Coordinator stated she was not aware of the new Schizophrenia diagnosis and therefore had not screened or referred the resident to the local mental health authority. Staff interviews also reflected that the resident had behavior issues when triggered, stayed in his room most of the time, and could voice his needs. The Regional MDS Coordinator stated the diagnosis came from the psychiatrist and that a new screening would be required, but it had not been done after the diagnosis was entered. The facility policy stated that all admissions must have the appropriate PASRR completed and that the administrator is accountable for monitoring the process.
