Food Storage, Labeling, and Sanitation Failures
Summary
Food was not stored, prepared, cleaned, or sanitized in accordance with sanitary standards in the kitchen and food storage areas. During observation, a kitchen staff member was preparing food without the required beard covering, and the Dietary Services Manager stated the beard cover should have been worn before continuing meal preparation. The kitchen floors and dry storage room floors were observed with black debris buildup, exposed concrete where floor coating was missing, and raised floor coating with debris underneath. The Dietary Services Manager stated he had been in the position for one month and was working with staff on daily kitchen cleaning, and also stated the facility was aware of the floor condition and had plans for a kitchen renovation within the next year. Opened containers of teriyaki glaze and barbecue sauce that required refrigeration after opening were found stored on open shelves in the dry storage room. The Dietary Services Manager reviewed the labels and confirmed the items should have been refrigerated after opening and needed to be discarded. Raw poultry marinating in a stainless steel container was stored in the refrigerator without a labeled preparation date or accurate expiration/use-by date. An opened bag of bagels and an opened bag of bread were also found resealed without labels showing the date opened or a use-by date, and the Dietary Services Manager stated both items should have been labeled when first opened and needed to be discarded. The June 2026 Kitchen Cleaning and Prep Log had missing entries for June 3, 4, 10, and 11, including no recorded verification of cleaning and sanitizing the griddle, steam table, cook prep table, cook prep sink, can opener, and knives on June 11. A kitchen staff member used an expired paper test strip dated 6/01/23 to test the sanitization solution in the red buckets, and the Dietary Services Manager stated expired strips were not appropriate and the solution needed to be retested with non-expired strips. Plastic food bins used for meal preparation were observed stacked while still wet after handwashing and placed on an unclean sink surface. In addition, the dishwasher had not been tested for the required minimum water temperature and sanitization solution concentration prior to first use on the day shift after breakfast, and the Registered Dietician confirmed testing and documentation were required at the beginning of each shift.
