Summary

The facility failed to assess and implement interventions to ensure a resident received necessary care and services to prevent avoidable pain during transport to a medical appointment. The resident had the capacity to understand and make decisions and was described by staff as a maximum-assist resident who required a Hoyer lift for transfers because of fractures and pain. CNA staff stated the resident could not tolerate more than 10 to 15 minutes in a wheelchair after therapy due to extreme pain, while OT staff stated the resident could sit in a wheelchair for about 45 minutes during therapy but would ask to return to bed afterward because of pain. The case manager informed the resident about the upcoming physician appointment and did not consult therapy to determine the best mode of transportation because the resident was alert and agreed to wheelchair transport. The resident later stated he could only tolerate about 30 minutes in a wheelchair due to severe pain and said he was in such severe pain at the appointment that his daughter had to retrieve his pain medication and administer it to him. The DON stated the transportation company refused to bring the resident back to the facility because he could not safely remain in the wheelchair without sliding out, and family reported that the resident was falling out of the wheelchair at the appointment and had to be propped up with a chair to keep him from sliding out.
