Summary

The facility failed to ensure one resident received necessary care and services to prevent accident hazards when the resident was transported in a shower chair unsafely and slid off the chair, resulting in a fall. The facility's policy stated residents are to be handled and transferred safely to prevent or minimize risk for injury. The resident's MDS showed the resident required partial moderate assistance for sit-to-stand and was dependent for tub/shower transfers. On 7/11/26, the resident had a witnessed fall while being pushed in a shower chair when the wheel became stuck on a metal frame on the floor. The resident reported generalized pain rated 7/10, and later documentation noted buttock pain, bilateral knee pain, and left shoulder pain. The resident denied hitting the head or losing consciousness and was taking Eliquis. CNA 1 stated she was transporting the resident to another shower room because the shower near the resident's room was not functioning, and while pushing the shower chair forward through the emergency doors, the wheel got stuck on the metal plate and the resident slid forward and fell. LVN 1 observed the resident on the floor and saw the shower chair wheel stuck on the metal plate. A later observation showed the DSD had CNA 2 demonstrate transport over the metal plate by pulling the shower chair backward, which the DSD stated was the best practice.
