PASRR Level II recommendations not incorporated into care plans
Summary
The facility failed to incorporate PASRR Level II recommendations into the assessment and care planning for two residents. For Resident 19, the initial PAS showed a Level 1 positive result and the Level II determination included multiple specialized add-on services, including mental health rehabilitation activities, ADL training/reinforcement, supportive services, psychotherapy/counseling, psychiatry consultation and follow-up care, safety monitors, pharmacy consultation, pain services consultation, ophthalmology consultation, PT, OT, speech therapy, dietary consultation, social services consultation, continence evaluation, and accessibility accommodations. The resident’s care plan dated 5/26/26 did not show an individualized care plan for the PASRR Level II recommendation. Resident 19’s record also showed a history of bipolar disorder and an MDS assessment with a BIMS summary score of 8, indicating moderately impaired cognition. During an interview and concurrent record review, the MDS Coordinator was unable to show a personalized care plan for the PASRR Level II recommendation from the DHCS psychologist. The Assistant Administrator and DON were informed of these findings and acknowledged them. For Resident 67, the PASRR Level II mental health evaluation identified recommended specialized services including mental health rehabilitation activities, ADL training/reinforcement, supportive services, psychotherapy, neuropsychology consultation, psychiatry consultation, safety monitors, pharmacy consultation, and pain services consultation. After the resident was readmitted, the facility’s plan of care records did not show a care plan problem addressing the PASRR Individualized Determination Report. RN 3 verified that a care plan had not been developed to address the PASRR recommendations, and the DON stated that the PASRR should be reviewed for accuracy and followed up if unable to complete.
