Incomplete and Inaccurate Controlled Medication Records
Summary
The facility failed to keep accurate and complete controlled medication records for residents receiving scheduled narcotic medications. During interviews and record review, the Medical Record Director, Director of Nursing, and an LVN described a system in which scheduled medications were delivered with a Shipping Manifest and a corresponding Controlled Drug Record (CDR), the medication was stored in the cart, removals were documented on the CDR, and administrations were documented on the MAR. The DON stated that all scheduled medication records were kept in her office, but when asked to provide Shipping Manifests and CDRs for the review period, multiple CDRs were identified that did not have corresponding Shipping Manifests available in the facility records. The missing Shipping Manifests were identified for residents 1 through 13 and included multiple orders for oxycodone, hydrocodone-acetaminophen, and morphine sulfate. The DON acknowledged that the facility did not have the corresponding Shipping Manifests for those CDRs. The facility also had inaccurate documentation when the DON compared CDRs to MARs for residents 11, 14, 15, 16, and 17. For resident 14, the CDR removals did not match MAR administrations on several listed dates and times. For resident 15, the CDR removals did not match MAR administrations on multiple listed dates and times. For resident 16, one CDR removal did not have a corresponding MAR administration. For resident 17, several CDR removals did not have corresponding MAR administrations. For resident 11, one CDR removal did not have a corresponding MAR administration. The facility policy for controlled substances required controlled substance inventory to be monitored and reconciled, and the system for reconciling receipt, dispensing, and disposition of controlled substances to include medication administration records and declining inventory records. The DON reviewed the policy and acknowledged it required the documentation to be accurate and monitored and reconciled. The Consultant Pharmacist reports from 9/23 through 3/24 were also reviewed, and the DON stated the reports did not document scheduled medication issues with Shipping Manifests, CDRs, and MARs. The DON stated it was the facility's expectation that issues with scheduled medications would have been identified.
