Excess Gravy Served Beyond Menu Portion

View Park Convalescent CenterLos Angeles, California Survey Completed on 07-02-2026

Summary

The facility failed to follow the menu and did not meet the nutritional needs of residents when additional gravy was added to Swedish meatballs instead of the 1-2 oz of gravy listed on the menu spreadsheet. The menu spreadsheet titled Summer Menus for regular texture diets listed Swedish meatballs, 1-2 oz gravy, egg noodles, vegetables, fruit, a wheat roll, margarine, raspberry parfait square, and milk as the lunch meal components. During lunch tray-line service, [NAME] 1 was observed using a 2 oz ladle to serve gravy and then adding more gravy while scooping the meatballs with a serving spoon. In a concurrent observation and interview, the Registered Dietitian stated [NAME] 1 was putting a bit too much gravy on the plate and that the resident menu is designed to meet nutritional needs; the RD also stated that excess gravy on the plate could potentially lead to weight gain if the practice continues. The facility's policy stated menus are to be prepared as written using standardized recipes, and the standardized recipe for Swedish Meatballs directed serving 2 meatballs with 1-2 oz gravy over egg noodles.
