Medication administration and controlled drug accountability failures
Summary
The facility failed to ensure safe medication administration for a resident with anemia and dementia who was admitted with a gastrostomy tube and lacked capacity to understand and make decisions. The resident had an order for ferrous sulfate oral solution 300 mg/5 ml, 5 ml via G-tube daily for anemia. During observation, the LVN stated she would have to wait and clarify the order, and later the DON and central supply staff stated the facility did not have ferrous sulfate 300 mg/5 ml available and had no record that it had been ordered. The DON stated that if the exact dose ordered was not available, the licensed nurse should have called and informed the physician. Review of the MAR showed the ordered ferrous sulfate 300 mg/5 ml was documented as administered repeatedly from April through June, and the DON stated that because the facility did not have the ordered medication available, the nurses had been administering ferrous sulfate 220 mg/5 ml instead and documenting it as the ordered dose. The facility also failed to ensure accurate accountability of controlled medications for a resident with peripheral autonomic neuropathy, paresthesia of skin, anesthesia of skin, and osteoarthritis who was cognitively intact and able to make decisions for activities of daily living. The resident had an order for hydrocodone with acetaminophen 5 mg/325 mg, one tablet by mouth every 6 hours as needed for severe pain due to leg neuropathy. Review of the controlled drug record showed a skipped line and an out-of-sequence entry: line 20 documented a dose removed on 6/30/2026, line 19 documented a dose removed on 7/1/2026, line 18 was left empty, and line 17 documented a wasted dose on 6/29/2026. The LVN stated a nurse skipped a line and was not sure how to mark the next dose after the line was skipped. The DON stated the controlled drug record must be used chronologically and lines must not be skipped. The facility also failed to document disposal of noncontrolled medications in the presence of a licensed nurse and a witness. During inspection of the medication room, the noncontrolled medication disposal log contained only one licensed nurse's initials or signature for disposals dated between 3/2026 and 6/2026. The LVN stated the log was missing a second licensed nurse or witness. The DON stated that two nurses must perform noncontrolled medication disposal together and that one nurse cannot sign as their own witness. The facility policy stated that noncontrolled medication destruction occurs in the presence of two licensed nurses and that witness signatures are entered on the medication disposition form.
