G-tube Medication Administration Not Performed as Ordered
Summary
The facility failed to follow physician orders and its own policy for enteral tube medication administration for three residents who received medications through G-tubes. The report states that the deficient practices involved not ensuring the prescribed flushes were given before, after, and between medications, and not administering medications in the manner ordered or consistent with manufacturer instructions. The deficiency was identified through observation, interview, and record review during medication passes. For one resident with severe cognitive impairment and total dependence for ADLs, the record showed orders to flush the G-tube with water between medication administrations and to give fluids pre- and post-medication administration. During observation, an LVN crushed two medications, added water to the cups, flushed the G-tube with 30 ml of water, and then administered the medications while undissolved medication remained in the cups. The LVN then stated there was no need for a final flush because she had already given water, but later acknowledged that a final flush was needed to ensure all medication entered the stomach and that she should have done so after the remaining medication in the cups was observed. For another resident with dementia and severe cognitive impairment, the record showed orders for G-tube flushes between medications and permission to give fluids pre- and post-medication administration. During observation, an LVN prepared five crushed medications, added water to each cup, and then gave an initial flush of 60 ml, flushed 30 ml to 40 ml between each medication, and gave a final 30 ml flush. The resident coughed and pushed fluid back into the syringe, and the LVN stated she had given a large amount of water to prevent the G-tube from clogging. The LVN later acknowledged she had not worn a gown during the G-tube medication administration and stated she should have. For a third resident with a G-tube, dementia, and total dependence for ADLs, the record included orders to dilute potassium chloride before administration, flush with water between medications, and give fluids pre- and post-medication administration. During observation, an LVN crushed tablets and opened capsules, mixed them with small amounts of water, and administered them via G-tube with 5 ml to 10 ml flushes between medications. The LVN also used the syringe plunger to push tamsulosin and LiquaCel through the G-tube and gave undiluted potassium chloride via G-tube, followed by a 10 ml flush. Medication remained in the cups after administration, and the LVN stated the resident had not received the complete doses of several medications, including the blood pressure medications and potassium chloride. The DON also observed that the resident did not receive all of the medication remaining in the cups.
