Failure to Follow Up on Audiology Evaluation for Resident With Hearing Loss

Muskego Health And Rehabilitation CenterMuskego, Wisconsin Survey Completed on 07-09-2026

Summary

The facility did not ensure a resident with hearing impairment received proper treatment and assistive services, including follow-up for an audiology evaluation. The resident was admitted with diagnoses including hemiplegia and hemiparesis following cerebrovascular disease, depression, major depressive disorder, and anxiety disorder. The resident’s quarterly MDS dated 5/18/26 indicated a BIMS score of 14 and documented no hearing concerns, but the resident later told the surveyor that hearing had decreased and that a hearing test had been planned but could not be completed because of extreme wax buildup in both ears. Record review showed an audiology referral was made on 10/22/25 due to decreased hearing, and the audiology exam on 10/25/25 could not be completed because wax was so deep in both ears that it needed to be removed before testing could occur. The resident reported that the wax had been removed but there had been no follow-up audiology assessment afterward. The surveyor could not locate a care plan for the resident’s hearing or audiology needs in the medical record. Additional record review showed that on 6/5/26 the NP documented cerumen impaction in both ears and used Debrox to soften the wax, with occasional ear discomfort noted. The surveyor found no documentation that the wax buildup had been addressed before that date or that the audiology consult recommendations were followed up after the initial failed exam. Interviews with the SW, MR, MDS nurse, CNA, and NHA confirmed the resident remained hard of hearing, that audiology recommendations were not clearly followed through, and that the facility did not have documentation of audiology assessment follow-up for the resident.
