Ineffective Pain Management and Incomplete Follow-Up for Resident With Severe Pain

Summary

The facility did not ensure effective pain management for a resident with recent back surgery, postoperative lumbar wound infection, chronic low back pain, and multiple pain-related diagnoses. The resident had intact cognition and was identified as being at risk for pain and receiving scheduled pain medication. The care plan noted pain related to the recent back surgery and included an intervention to notify the physician if pain interventions were unsuccessful or if the resident’s pain complaint changed significantly from baseline. The resident’s physician ordered multiple pain medications, including oxycodone-acetaminophen PRN, tizanidine PRN, acetaminophen PRN, diclofenac gel PRN, a fentanyl patch, Lidoderm patch PRN, and Lyrica three times daily, along with pain evaluation every shift. The MAR showed repeated pain scores, including several ratings of 7 to 10, and documented administration of oxycodone-acetaminophen and one fentanyl patch dose. However, Lyrica was not administered from 5/19/26 through 5/22/26 because it was unavailable, and the resident did not receive additional PRN medications such as tizanidine, acetaminophen, diclofenac, or Lidoderm for breakthrough pain. The record also showed gaps in pain medication administration during the overnight hours, and one documented follow-up pain assessment after PRN medication was missing. The resident’s significant other reported that pain medication was not given as needed, especially at night, and stated the resident had to wait about 2 hours for pain medication on one occasion. A grievance report documented the concern that the resident requested pain medication and waited almost 2 hours before receiving it. Staff interviews reflected differing accounts about when medication was given, and one LPN later stated the MAR did not support the claimed administration time. The interim DON stated there was no documentation that the provider was updated about the resident’s continued complaints of pain, and acknowledged that nurses should monitor pain and update the provider if pain was not effectively managed.
