Summary

The facility did not ensure the resident environment remained as free of accident hazards as possible for five sampled residents. One resident with a diagnosis of delusional disorder and moderately impaired cognition repeatedly expressed a desire to leave the facility and made multiple attempts to do so, including attempts to exit the building, go into the road, and leave against medical advice. The resident also barricaded themself and a roommate in a room, became agitated, and made statements that staff were holding them captive. Although the resident had an elopement evaluation that initially found no risk, the record did not show a reassessment after the repeated attempts to leave, and the care plan was not updated with safety measures or redirection interventions after these events. The same resident continued to verbalize a desire to leave and stated they had nowhere to go, but discharge planning was not begun despite the ongoing statements and attempts to leave. Staff documented multiple episodes in which the resident wanted to leave, signed AMA paperwork, asked for medications and belongings to prepare to leave, and attempted to exit through facility doors. Surveyor observation also found the facility entrance unlocked during the night shift, and the entrance was not visible from the nurses’ station. Facility leadership confirmed the door should have been locked and that the resident did not wear a WanderGuard. The medical record also did not contain safety interventions directing staff how to redirect or approach the resident when agitated or attempting to leave. The facility also did not have current smoking assessments for four residents who smoked or vaped. One resident with quadriplegia had a care plan indicating vaping and a smoking assessment dated before the most recent quarterly MDS. Another resident with CHF had a care plan indicating smoking, but the facility had no smoking assessment on file. A third resident with a history of nicotine dependence had a care plan indicating smoking and vaping, but the most recent smoking assessment was dated before the latest quarterly MDS. A fourth resident with tobacco use and moderate cognitive impairment also had an outdated smoking assessment. The DON confirmed that the assessments for three residents were late and that one resident did not have a smoking assessment.
