Summary

The facility did not ensure a resident with hypertensive heart disease with heart failure, paroxysmal atrial fibrillation, an automatic cardiac defibrillator, COPD, and chronic combined systolic and diastolic heart failure received care and treatment in accordance with the plan of care when the resident’s blood pressure was outside ordered parameters. The resident had physician orders for daily blood pressure checks and was under the care of a cardiologist, but the medical record did not contain a repeat blood pressure, documentation that the physician was notified, or instructions for monitoring after a blood pressure of 78/55 mm/Hg was recorded. The resident’s record also did not contain a cardiac-specific care plan, and the care plan did not include instructions for what to do if blood pressure was outside the ordered parameters. Facility staff interviews confirmed the missing documentation and follow-through. The ADON stated the resident had significant edema and had been treated with Lasix while kidney function was monitored, and the DON stated a physician should be contacted for a critical blood pressure level and confirmed the resident’s blood pressure was 78/55 mm/Hg. The UC recalled that NP-G may have been notified and said they were told to monitor for symptoms such as lethargy, nausea, light-headedness, and palpitations and to call cardiology for any symptoms, but this was not documented in the medical record or 24-hour nursing report. NP-G stated that if notified, they would have instructed staff to obtain a follow-up blood pressure, monitor for symptoms, and include the cardiologist, but the record did not show that these actions occurred.
