Summary

Resident 2 had memory impairment, altered mental status, a mood disorder, and used a wheelchair. Resident 2’s record also did not contain documentation of the incident. Staff C stated they were told about the event but did not investigate further and did not document it in the resident’s record. Staff A stated the incident was thought to be accidental and not intentional, so no further interventions were put into place and no full investigation was conducted, even though staff acknowledged that potential abuse incidents should be investigated and documented.
