Failure to control GI/Norovirus outbreak and follow contact precautions
Summary
The facility failed to provide and implement an infection prevention and control program during a gastrointestinal illness and Norovirus outbreak. The report states that the facility did not consistently follow GI Contact Precautions for multiple residents with symptoms of nausea, vomiting, and diarrhea, including failure to wear gown and gloves and failure to perform hand hygiene with soap and water when entering resident rooms. Staff were observed entering rooms with symptomatic residents without proper PPE, moving between resident rooms without changing masks, and in some cases using hand sanitizer instead of soap and water during the outbreak. The facility also did not consistently surveille resident and staff illness, and the outbreak line lists were incomplete, with missing onset and resolution dates and residents absent from the resident log despite documented symptoms in the medical record. The facility also failed to cohort or isolate symptomatic residents and protect exposed residents. Resident 1, who had COPD and was dependent on staff for ADLs, lived with Resident 24, who developed poor appetite, nausea, and vomiting. Resident 1 later developed vomiting and required Geri-lanta, and said the roommate became ill first and that they were not offered a different room. Resident 2, who had a stroke and was dependent on staff for ADLs, shared a room with Resident 17, who had diarrhea; Resident 2 later developed vomiting and diarrhea and required loperamide. The report also describes other roommate pairs, including Residents 15 and 34, where the asymptomatic roommate was not moved despite available empty rooms. Staff acknowledged they did not follow the facility policy for moving roommates or cohorting residents during the outbreak. The facility failed to notify the local health jurisdiction, residents, families, and the public in a timely manner. The local health jurisdiction stated it was notified on 07/13/2026, although the facility had identified an outbreak earlier. The facility did not post a visitor notice at the entrance until after the outbreak had already been underway for several days, and family members reported they were not informed when visiting residents who were ill. The report also states the facility failed to remove Resident 1 from isolation in accordance with facility policy. Staff interviews showed confusion about outbreak procedures, including when to use masks, when to change them, and when staff could return to work, and the staff surveillance log did not identify trends or patterns among ill staff.
