Unclear pain orders, missed ordered care, and obsolete medication monitoring orders
Summary
The facility failed to ensure nursing services were provided within professional standards of nursing by not clarifying physician orders when required for pain management. Resident 32 had diagnoses including a chronic autoimmune disease of the central nervous system, disc degeneration, and migraines, and the MDS noted mild pain that occasionally affected sleep and daily activities. The resident had two as-needed pain medication orders, including an over-the-counter medication and an opioid medication, but the opioid order did not show how long to wait between doses and neither order included parameters to guide nurses on which medication to give for what level of pain. The DON stated the orders needed clarification because they were unclear. The facility also did not follow physician orders for two residents’ air mattresses and one resident’s weekly weights. Resident 6 had traumatic spinal cord dysfunction, frequent severe pain, and two pressure wounds with full thickness skin and tissue loss and exposure of bone or tendon. An order directed that the resident’s air mattress be kept on the soft setting, but observations on multiple occasions showed the mattress set three quarters of the way toward firm, and the DON confirmed it was not kept at the ordered setting. Resident 54 had diagnoses including a progressive blood circulation disorder, malnutrition, and difficulty controlling blood sugar, and was at risk for pressure wounds. An order directed the air mattress to be set on the soft setting each shift, but observations showed it on the firmest setting on multiple occasions, and the DON confirmed it was not kept as ordered. Resident 10 had alcoholic liver cirrhosis, hypertension, and diuretic use, and the nutritional risk care plan directed staff to monitor and report significant weight loss. A physician’s order required weekly weights every Wednesday, but the last documented weight was obtained on 05/13/2026 and the record showed missed weekly weights. The dietitian and DON both confirmed the weekly weight order was not followed. The facility also left an obsolete order in place for Resident 29, who had high blood pressure, a history of stroke, and an enlarged heart. The resident had switched from an anticoagulant to an antiplatelet medication, but the order to monitor for anticoagulant adverse side effects remained active even after the anticoagulant was discontinued.
