Accident hazards and inadequate supervision
Summary
The facility failed to ensure the environment was free from accident hazards and that residents received adequate supervision to prevent accidents. Surveyors observed multiple sharps containers in rooms on Birch Hall and Cedar Hall, as well as on treatment carts, filled above the fill line. Staff C, the Assistant Director of Nursing, confirmed the containers were overfilled and stated they should be replaced before they were full to prevent infections and provide a safe space for staff when disposing of sharps after use. For Resident 63, who had severely impaired cognition, used a wheelchair, and had a seizure disorder, the record showed an order for a self-releasing seat belt to be used in a tilt-in-space wheelchair. A safety device assessment was completed for the belt, but there was no corresponding assessment for the tilt-in-space wheelchair itself. The resident was observed ambulating in the wheelchair with the belt across the lap, and the Resident Care Manager confirmed no assessment had been completed for the wheelchair. Resident 67, who had dementia, restlessness and agitation, age-related physical weakness, required supervision with all activities of daily living, and had two falls during the assessment period, had a fall care plan that identified poor safety awareness as a risk. After the falls, the record and staff interviews showed no new care plan interventions were developed or implemented beyond non-skid socks and encouraging use of a wheelchair instead of a walker. Resident 67 was later observed getting up from a chair in the hallway and walking back to the room without staff assistance or supervision, and the DON stated there should have been care planned interventions after the falls. Resident 17, who was an active smoker, stated smoking materials were kept in an unlocked closet or carried in a pocket, and no lock box was available. The resident said confused residents wandered into the room at times. The smoking policy required residents who kept their own smoking materials to store them in a locked cabinet when not in use, but staff were not aware that locked storage was unavailable for this resident. Resident 94, who had advanced dementia, delirium, agitation, restlessness, impulsivity, and a recent hip fracture after a fall, was admitted with no fall risk or elopement risk identified on the nursing admission assessment. The record showed the resident had been wandering, trying to leave the facility, and was placed on one-to-one supervision on the night shift. Despite this, the resident exited through the therapy door without staff knowledge or supervision and was found outside on the ground after an unsupervised fall. Staff later stated the resident should have been identified as both a fall risk and an elopement risk with an individualized care plan and wander guard device in place. Resident 28, who had encephalopathy, stimulant dependence, chronic respiratory disease, failure to thrive, paranoia, anxiety, wandering behaviors, and a history of smoking, had an elopement care plan and a wander guard device that had been moved from the ankle to the wheelchair after the resident cut it off. An elopement assessment described the resident as confused, exit seeking, and independent with mobility. The incident investigation showed the resident was seen outside two buildings away from the facility while smoking a cigarette, and the wander guard did not alert staff. Staff stated the resident was not safe to be outside unsupervised and was not assessed to smoke safely without supervision.
