Summary

The facility failed to ensure that residents who were trauma survivors received trauma-informed care for 2 of 3 residents reviewed for trauma-informed care. The facility policy stated residents who were trauma survivors would be screened on admission, triggers would be identified with the resident and interdisciplinary team, and resident-centered care plans would reflect trauma-related needs and preferences. Review of the records for Resident 7 and Resident 65 showed that both had diagnoses including PTSD and depression or mood disorder, but their care plans did not list trauma-related triggers or person-centered interventions related to their trauma histories. Resident 7’s record showed a diagnosis of PTSD, major depressive disorder, and psychophysiologic insomnia, and the resident was dependent on one to two staff members for ADLs with moderately impaired cognition. A post-traumatic checklist documented that watching specific television shows was stressful and overwhelming because it reminded the resident of past negative experiences, and the resident later stated that movies or television scenes related to a past sexual assault triggered memories and stress, while coloring helped distract them. Resident 65’s record showed diagnoses including mood disorder, depression, and PTSD, with intact cognition and assistance needed with ADLs. The resident stated their trauma began at age two and continued through adolescence, and identified triggers such as clowns and women with lots of makeup, but the care plan did not include those triggers or interventions. Staff K stated they were unaware of residents who were trauma survivors, and Staff H stated the trauma-informed care process should include an evaluation on admission, readmission, and quarterly, with the care plan showing a synopsis of the trauma, triggers, and interventions; Staff H stated the process was not followed.
