Summary

The facility failed to ensure restorative nursing services were consistently provided to 2 of 4 residents reviewed for ROM services. The facility policy titled, Restorative Nursing Programs, stated the program was intended to help residents maintain their highest practicable functional levels and prevent unnecessary declines. Resident 4 was admitted with diagnoses including left hemiplegia, muscle weakness, difficulty walking, and reduced mobility. The resident’s assessment showed cognition was intact and the resident was dependent on staff for care and mobility. In interview, Resident 4 stated insurance stopped in March 2026 and they had not received therapy since then, and later stated they had not walked with staff since March 2026. The care plan showed limited physical mobility related to weakness and deconditioning and that the resident required supervision or touching assist by one staff member to walk. Resident 31 was admitted with diagnoses including kidney failure, vision loss, and a left knee amputation with a left leg prosthesis. The resident’s assessment showed moderately impaired cognition and dependence on staff for care and mobility. Resident 31 stated it had been six weeks since the last therapy session and reported losing strength in the legs and feeling like they did when first admitted. During observation, the resident was sitting in a wheelchair in the hallway and stated they were doing upper body stretching and exercises because they had no therapy. Staff K stated nursing assistants did not have time to do a real restorative program and only tried to do the best they could. Staff G, the DOR, stated the restorative nursing program had not been consistently provided and had been inactive for approximately one month. Staff A, the Administrator, acknowledged inconsistency with the restorative program.
