Incomplete and Late Documentation of Vital Signs and Skilled Assessments

Summary

The facility failed to maintain complete and accurately documented medical records for 2 residents reviewed. For one resident, the record showed multiple instances in which RN B, LVN C, and LVN D documented vital signs on the MAR that matched prior entries exactly, including repeated blood pressure, temperature, pulse, respiratory rate, and oxygen saturation values across different shifts and dates. The resident’s orders included monitoring temperature and oxygen saturation for symptoms, GI symptoms every shift, and pre- and post-nebulizer treatment assessments every 6 hours, yet the vital sign summaries did not consistently contain corresponding entries for the same time periods. The first resident was an elderly female admitted with anemia, GI hemorrhage, gastrostomy status, unspecified protein-calorie malnutrition, dehydration, and dementia. Her discharge/return not anticipated MDS reflected a blank BIMS score, and she remained in the facility from 06/19/26 to 06/24/26. The MAR showed repeated documentation of identical vital signs by different nurses on consecutive shifts, including the same BP, temperature, pulse, respirations, and O2 saturation recorded on separate days. For the breathing treatment documentation, multiple sets of pre- and post-treatment vital signs were entered as exactly the same, including lung sounds, pulse, respirations, and O2 saturation, with several entries documented in the same manner across the resident’s stay. For the second resident, the facility failed to document Skilled Assessments accurately and timely on multiple occasions. The resident was an elderly male admitted with lumbar vertebra fracture, COPD, cognitive communication deficit, anemia, hypertension, and hyperlipidemia, and his admission MDS showed a BIMS score of 13. The record review showed numerous Nursing Advanced Skilled Evaluations and one Change in Condition Evaluation that were created hours to days after the effective date, including documentation delays ranging from a few hours to more than 75 hours. One skilled evaluation also had an incomplete vital sign entry with no respiratory rate documented. The record further showed that no Nurse Advanced Skilled Evaluations were done on several dates during the review period.
