Failure to Complete Required Smoking Safety Evaluations
Summary
The facility failed to establish and implement smoking policies and smoking safety evaluations in accordance with its own policy for five residents who smoked. The report states that the facility did not complete required smoking evaluations on a quarterly basis or after changes in condition for Residents #19, #22, #60, #58, and #07, despite each resident having a smoking preference and being identified in care plans as at risk for injury related to smoking. Resident #19 had diagnoses including Alzheimer's disease, Huntington's disease, behavioral disturbance, psychotic disturbance, mood disturbance, and anxiety, and his BIMS score reflected severely impaired cognition. His care plan identified him as at risk due to smoking and called for evaluation and monitoring of safe smoke practices, including dexterity and coordination. The smoking evaluation on record stated he had hand tremors and decreased finger dexterity and required supervision while smoking, yet the facility did not show additional quarterly evaluations after the one completed on 01/31/26. During observation, he was smoking outside in the designated area with staff supervision and moderate assistance, and staff provided the cigarettes and smoking paraphernalia. Resident #22 had diagnoses including dementia, psychotic disturbance, mood disturbance, anxiety, lack of coordination, and muscle weakness, with moderately impaired cognition. His care plan identified smoking-related injury risk, and the smoking evaluation stated he required supervision because he had a history of smoking in non-designated areas and was unable to safely light and extinguish smoking materials. The record showed no smoking evaluations after 09/09/25. During observation, he was smoking in the designated area with staff supervision and minimal assistance, and he told staff did not allow him to keep cigarettes and a lighter with him even though he wanted to do so and believed he could smoke unsupervised. Resident #60 had legal blindness, severe obesity, muscle weakness, difficulty walking, and lack of coordination, with intact cognition. His care plan identified smoking-related injury risk, and the record showed one annual smoking evaluation on 10/02/25 that found him to be a safe smoker, with no later evaluations documented. During observation, he was smoking unsupervised in the smoking area with Resident #58 and stated he was allowed to keep cigarettes and a lighter with him. Resident #58 had hemiplegia and hemiparesis following cerebral infarction, nicotine dependence, difficulty walking, and lack of coordination, with intact cognition. Her care plan also identified smoking-related injury risk, and the record showed one quarterly smoking evaluation on 01/14/26 with no later evaluations documented. During observation, she was smoking unsupervised in the smoking area with Resident #60 and stated she and Resident #60 were allowed to keep cigarettes and lighters with them and smoke independently. Resident #07 had diagnoses including dementia, psychotic disturbance, mood disturbance, anxiety, amnestic disorder, PTSD, muscle weakness, difficulty walking, lack of coordination, unsteadiness on feet, and age-related cognitive decline, with a BIMS score indicating intact cognition. His care plan did not reflect his smoking preference or smoking-related injury risk, and the record showed one smoking evaluation on 09/19/25 stating he needed supervision because he had fallen asleep while smoking. No later smoking evaluations were documented. During interview, he stated the facility scheduled smoking times and stored cigarettes at the nursing station, and he said he followed the safety rules while smoking. The ADON and DON both stated that periodic smoking evaluations were important and acknowledged that some evaluations had not been completed quarterly, while the DON was uncertain whether the evaluations had been completed or whether the system failed to populate the data correctly.
