Failure to Complete PASARR Specialized Service Referrals
Summary
The facility failed to incorporate the recommendations from PASARR level II determinations and PASARR evaluation reports into assessment, care planning, and transitions of care for 2 residents. For both residents, the facility did not complete and accurately request PASARR specialized services within the 20-day timeframe after the Interdisciplinary Team meeting on 4/20/26. The report states this failure could place residents at risk of not receiving needed treatment and services. Resident #1 was a female admitted with a diagnosis of Mild Intellectual Disabilities and had a quarterly MDS assessment showing a BIMS of 8 and moderate cognitive deficit. Her care plan identified her as PASARR positive and referenced specialized services, including habilitation coordination services through Intellectual and Developmental Disabilities Specialized Services. Her habilitative service plan dated 4/28/26 approved PASARR services including a customized wheelchair and therapy assessments and services for PT, OT, and speech therapy. During interview and observation on 7/23/26, she was unable to answer questions coherently due to her intellectual functioning and was observed in therapy. Resident #2 was a male admitted with a diagnosis of Mild Intellectual Disabilities and had a comprehensive MDS assessment showing he was unable to complete a BIMS because he was rarely understood. His care plan identified him as PASARR positive and referenced specialized services. The Interim DOR stated the residents had not received PASARR services because the therapy evaluations were not completed after the initial Interdisciplinary Team meeting, and that the delay affected the customized wheelchair process for Resident #1. MDS Nurse Coordinator A stated she could not find referrals in the portal for either resident and noted that if residents were not referred appropriately for PASARR services, they could have missed out on state services they may need.
