Pest Control Program Ineffective With Flies in Resident Areas and Kitchen

Fallbrook Rehabilitation And Care CenterHouston, Texas Survey Completed on 07-03-2026

Summary

The facility failed to maintain an effective pest control program, and survey observations found live flies in multiple areas of the building, including Hall 100, Hall 200, and one resident room. In the resident room, a fly was observed flying in the room and landing on the resident’s head before exiting into the hallway. The resident was a cognitively intact female with chronic respiratory failure with hypoxia and end-stage renal disease, and her care plan noted enhanced barrier precautions for a dialysis port and continuous oxygen at 3 LPM via nasal cannula. Additional observations showed a fly in Hall 200 near another resident’s room and approximately three flies flying in Hall 100. The resident in the room where a fly landed on her head stated that the facility had an issue with flies and that flies were always in her room, but she had not reported it. Another resident on Hall 200 stated that flies were always in the facility and in her room, and that she had not reported them. In the kitchen, the Dietary Manager was observed preparing bread for lunch and leaving it uncovered, after which a fly landed on the bread. The Dietary Manager swatted the fly away, discarded the bread, and performed hand hygiene. During interviews, staff stated that food should be covered immediately after preparation and that the kitchen should be free of flies. The Dietary Manager, DON, and Administrator all acknowledged that pest control serviced the facility monthly, and the Administrator stated he believed the current pest control system was effective even though flies were observed in the facility and kitchen.

Short Summary

Roach Infestation and Dining Room Sanitation Issues A resident reported seeing cockroaches in the dining room for about a year, and staff later observed roaches crawling in the dining room and near food stored in cabinets. Food debris, open cereal, and covered plates with meal remnants were found in the dining room, along with buildup, missing caulking, warped surfaces, and other areas where pests could be present. Staff also reported seeing roaches in the kitchen and dining room, while the facility had recently changed pest control vendors and had limited documented pest control follow-up.
