Main Dining Room Temperatures Not Maintained or Logged
Summary
The facility failed to provide a comfortable and homelike environment by not maintaining safe temperatures in the main dining room. Record review showed the facility’s ambient temperature requirement for resident and patient areas was 71 to 81 degrees Fahrenheit, and the temperature logs were incomplete, with some entries marked done on time but no action recorded on 04/18/2026 and 06/13/2026, and no records provided for 07/17/2026. The logs that were available did not include times, and some did not identify which dining room was checked. Resident #1 was a female with diagnoses including major depressive disorder and cognitive communication deficit, and her quarterly MDS showed a BIMS score of 15 out of 15. She reported that she used to eat in the main dining room but stopped because she would sweat during lunch and dinner due to the heat, and instead ate in her room because it was cooler. She said this made her feel lonely because she liked socializing in the dining room, and she also sweated while attending bingo there. She stated she had not told staff because she assumed they knew about the heat. Observations in the main dining room on 07/24/2026 showed ambient temperatures ranging from 79.7 to 81.1 degrees Fahrenheit, with the thermostat reading 80 to 82 degrees Fahrenheit while the cool setting was 71 to 72 degrees Fahrenheit. A confidential staff member reported the thermostat had read 80 degrees at lunch and above 81 degrees at dinner, and provided a photo showing the thermostat at 85 degrees on 07/21/2026 at 6:24 PM. A service request dated 06/25/2026 documented that the Makeup Air units in the common area dining hall were not cooling and that the work was not yet complete because a quote was needed. The Maintenance Director stated he was responsible for checking temperatures, could not find some of the logs, could not recall the times temperatures were checked, and said he had no staff checking temperatures when he was not present.
