PASARR Screening and Level II Assessment Not Completed Correctly
Summary
The facility failed to ensure that the PASARR Level I screening for a resident with diagnoses including major depressive disorder and a history of traumatic brain injury was completed accurately and in a timely manner, and the resident did not receive a PASARR Level II assessment as expected. Record review showed the resident was admitted with multiple diagnoses, including unspecified intracranial injury with loss of consciousness, flaccid hemiplegia affecting the left nondominant side, major depressive disorder, recurrent, unspecified, and a personal history of traumatic brain injury. The resident’s annual MDS reflected a BIMS score of 14 and non-traumatic dysfunction. The resident’s care plan included a PASARR DME focus area and noted that a customized manual wheelchair was requested during the initial IDT meeting. The PASARR Comprehensive Service Plan showed an IDT meeting occurred and the resident was approved for PT, OT, ST, and DME. An email from the PASRR NFSS Compliance Department stated the facility was out of compliance because HHSC PASARR records did not show a request for specialized services for the resident within the required timeframe after services were agreed to in the IDT meeting. During interviews, the MDS Coordinator stated she oversaw PASARR assessments and said a PASARR should be completed within 72 hours of a resident entering the facility. She stated major depressive disorder would indicate a positive Level I and that a new PASARR was generated because the resident’s previous PASARR from the prior facility was not correct. The DOR stated he handled assessments and therapies and described delays in obtaining the customized wheelchair due to provider availability and a delayed physician signature. The ADM and DON stated PASARR screening should be completed before admission and that a positive Level I would trigger further review and services, while also stating that an inaccurate PASARR could delay services.
