Summary

The facility failed to ensure that respiratory equipment was properly stored when not in use for three residents who were receiving oxygen and/or nebulizer treatments. Resident #4 had diagnoses including COPD, respiratory failure, and heart failure, and was ordered oxygen via nasal cannula every shift and PRN nebulizer treatment. During observation, the resident’s oxygen tubing was hanging on the wheelchair and the nebulizer mask was lying on the bedside table and not bagged while not in use. The resident stated that no staff had told her it needed to be bagged and believed it should be bagged for health reasons to prevent germs from getting in it. Resident #1 had diagnoses including COPD, acute respiratory failure, and pulmonary hypertension, and had a physician order for budesonide via nebulizer every 12 hours. During observation, the resident’s nebulizer mask was sitting on the nightstand and not bagged while not in use. The resident stated that nurses came and administered the medication and placed the mask on, but he was not sure whether it should be bagged. Resident #3 had diagnoses including unspecified foreign body in bronchus causing asphyxiation, heart failure, pneumonia, and pulmonary hypertension, and had an order for albuterol via nebulizer every 6 hours. During observation, the resident’s nebulizer mask was on the nightstand and not bagged while not in use, and the available bag in the room was dated several days earlier. The resident stated he thought it should be bagged to keep germs out, but was not sure why it was not bagged. Staff interviews confirmed that oxygen tubing and nebulizer masks were supposed to be bagged and dated when not in use, with nurses and CNAs responsible for ensuring this was done.
