Summary

The facility failed to ensure that resident rooms remained free of accident hazards for four residents reviewed for accident prevention. During observations on 07/27/26, scissors and an odor ban disinfectant spray were found in Resident #6’s room, a Tide Pods laundry detergent container was found in Resident #5’s room, Nystatin cream along with scissors and a safety pin were found in Resident #8’s room, and scissors with two nail clippers were found in Resident #3’s room. Resident #6 was a male with bipolar disorder and a BIMS score of 9, indicating moderate cognitive impairment. His care plan included monitoring for risk of harm to self or others, including possession of weapons or objects that could be used as weapons. During observation and interview, he stated he obtained the odor ban spray from Walmart because his room smelled bad and that he used the scissors to trim his beard. He also stated he was not supervised when he used them. Resident #5 was a female with bipolar disorder and a BIMS score of 11, indicating mild cognitive impairment. Her care plan addressed ADL self-care deficits. During observation and interview, a Tide Pods container was seen on her nightstand, and she stated it had been there since she moved in and that staff had placed her belongings there. Resident #8 had lack of coordination and needed assistance with personal care; his BIMS score was 14. During observation and interview, a safety pin, scissors, and Nystatin cream were on his nightstand, and he stated he had not been given instructions on how to use them. Resident #3 had dementia and intermittent explosive disorder with a BIMS score of 6, indicating severe cognitive impairment. During observation, scissors, two nail clippers, and bleed stop were found in a clear plastic drawer in his room, and he stated he was not sure how the items got there.
