Summary

The facility failed to ensure call lights were accessible to two residents, Resident #20 and Resident #80, both of whom had impaired mobility and significant cognitive impairment. Resident #20 was a male with diagnoses including acute and chronic respiratory failure with hypoxia, obesity, and acute pulmonary edema; his quarterly MDS showed a BIMS of 09 and dependence on all tasks. Resident #80 was a female with diagnoses including sequelae of cerebral infarction, hypertension, hyperlipidemia, and muscle weakness; her quarterly MDS showed a BIMS of 00 and dependence on all tasks. Their care plans reflected the need for assistance with ADLs, and Resident #80's care plan specifically included placement of the call light within reach. During observations and interviews, both residents' call lights were found on the floor. Resident #80 stated she did not know the call light was on the floor and said she had not been hurt because she could not reach it. Resident #20 stated he could not reach the call light on the floor and needed assistance with all ADLs and getting out of bed. Staff interviews indicated call lights should be within easy reach or close to the resident's hand, and staff stated they make rounds to check that call lights are within reach. The facility policy stated call lights should be placed on the bed at all times and never on the floor or bedside stand.
