Dirty Fingernails and Incomplete ADL Hygiene Care
Summary
The facility failed to ensure residents who were unable to perform activities of daily living received necessary assistance with grooming and personal hygiene, specifically nail care. During observation, Resident #7, Resident #12, and Resident #68 were found with dirty fingernails or visible brownish/blackish substance under or around their nails. The deficiency was identified during survey observation and interview, and the report states the facility failed to ensure these residents' nails were clean on 07/07/2026. Resident #7 was an older female with Parkinson's disease and anxiety disorder. Her MDS reflected moderately impaired cognition, and she required assistance with personal hygiene, dressing, showers, and toileting hygiene. Her care plan stated she required staff assistance with all ADLs, including hygiene and grooming. When observed in her room, blackish/brownish substance was noted under the fingernails of her right forefinger, middle, and ring fingers. When asked about nail cleaning, she stated she did not know and said she was unable to clean or trim her own fingernails because of Parkinson's disease. Resident #12 was an older female with type 2 diabetes mellitus with kidney complication, pain, and unspecified dementia. Her MDS showed moderately impaired cognition and substantial/maximal assistance was needed for personal hygiene, with supervision or touching assistance for showers and dressing. Her care plan stated she required staff assistance with showers, hygiene, and grooming. When observed in her room while waiting to go to dialysis, blackish/brownish substance was noted under the fingernails of both hands. Resident #68 was a 78-year-old female with dementia, osteoarthritis, and diabetes mellitus. Her MDS showed severe cognitive impairment, limited range of motion in both upper and lower extremities, and dependence on staff for bathing and personal hygiene. Her care plan stated she required staff assistance for all ADLs and bathing. When observed in the dining room, her left hand had a thick brown substance on the hand and fingers, with brown substance under the fingernails. An RN stated the hands looked dirty and could be BM, and a CNA later stated nail care should be done with baths but had not been completed that morning.
