Summary

The facility failed to develop and implement comprehensive care plans that reflected physician-ordered and individualized interventions for 3 of 9 residents reviewed. For Resident #9, who had COPD, urinary retention, moderate cognitive impairment, maximal ADL dependence, and an indwelling urinary catheter, the care plan included general catheter care and tubing/bag changes but did not include the physician-ordered interventions for the catheter secure device, checking placement of the dignity cover, keeping the bag below bladder level, or ensuring the bag did not touch or drag on the floor. The MDS Coordinator stated she had not reviewed the care plan before and acknowledged it should have included all physician-ordered interventions, including the Cath secure device. For Resident #53, who had CKD stage 2, DM, venous insufficiency, BPH, bladder dysfunction, essential hypertension, and mild cognitive impairment, the care plan identified a suprapubic catheter and BPH, but the resident was observed with the catheter hanging on the wheelchair arm above bladder level. The care plan also reflected that the resident refused a privacy/dignity cover because he wanted to see urine output, yet observations showed the catheter and drainage bag positioned above the level of the bladder while the resident was in bed and while sleeping. During an interview, the resident stated he told staff to place the catheter there, and later refused catheter care and said he did not have time for questions or catheter care. For Resident #99, who had an intracranial injury with loss of consciousness, spastic hemiplegia, hemiplegia, head injury, BPH, dysphasia, and hypertension, the care plan included allowing adequate time to urinate and a separate focus for urinary incontinence. However, the plan was not updated to reflect the resident’s actual catheter-related behavior described by staff, including hanging the catheter above the bladder on the side of the chair. The resident was observed in a geriatric chair after lunch wearing a brief and pants over the brief, and staff stated the care plan was general, not individualized, and did not accurately reflect the resident’s current needs and behaviors.
