Summary

The facility failed to maintain hot water temperatures below 120 degrees Fahrenheit in a resident bathroom, creating an accident hazard related to potential scalding. Resident 87 was admitted with dementia, had a BIMS score of 3 indicating severe cognitive impairment, and was independent with walking 10 feet. The resident was observed ambulating independently on the unit and used the bathroom for toileting and handwashing. During observation, the hot water in Resident 87’s bathroom sink felt hot to the touch and could not be kept on a bare hand for more than two seconds without discomfort. The Administrator confirmed the water was too hot for safe use by residents. The Maintenance Director stated the unit boiler was set at 135 degrees Fahrenheit and confirmed it needed to be lowered to 118 degrees Fahrenheit to reach a safe temperature. The Administrator also stated the facility did not have a policy or procedure for maintaining hot water temperatures at a safe level.
