Unsafe and Unsanitary Resident Environment
Summary
The facility failed to provide a safe, functional, sanitary, and comfortable environment for residents, staff, and the public. Surveyor observations and interviews identified rodent activity in the building, including a rodent seen under the baseboard heater in a resident’s room and a family report that a rat was seen in another resident’s room. The resident involved in the complaint was admitted with metabolic encephalopathy and COPD, had a BIMS score of 6 out of 15, and was dependent on staff for activities of daily living and transfers. The facility had a pest control agreement for bi-weekly rodent control, but the service reports showed trap-setting visits on only three dates, and the Administrator stated the visits had been reduced to about every three weeks because the rodent issue seemed to be improving. Surveyors also observed strong foul odors on 3 of 4 units on all days of the survey. The odors were smelled in hallways, resident rooms, and bathrooms, and staff acknowledged their presence. In addition, a resident with heart failure and a BIMS score of 11 out of 15 reported that staff had not made the bed for three to four days and had not washed the floor in the room. Surveyors observed a broken bedside table with exposed sharp edges and a sticky floor with discolored footprints, and these conditions remained on later observations. A nurse stated broken furniture should be entered in the maintenance log, but the maintenance log did not show the bedside table had been reported. Further observations showed additional room cleanliness concerns. One resident with schizoaffective disorder stated staff did not regularly wash the room floor, and surveyors observed cereal on the floor and a sticky surface. Another resident with paranoid schizophrenia had dark red and brown smears on the floor, including areas near the bathroom. Staff interviews showed the janitor cleaned common areas while housekeeping cleaned resident rooms and bathrooms daily, but the Maintenance Director and DON were unable to provide evidence that the facility had maintained a safe, functional, sanitary, and comfortable environment for residents as required.
