Significant medication errors with missed, omitted, and incorrect orders
Summary
The facility failed to ensure residents were free from significant medication errors related to medication reconciliation and administration after hospital transfers and during ongoing treatment. For Resident ID #48, who was readmitted with dementia and type II diabetes mellitus after hospitalization for acute cystitis, the hospital continuity of care form indicated insulin glargine was to be discontinued and cefpodoxime 100 mg every 12 hours was to be started. The resident continued to receive insulin glargine 22 units daily from 4/11 through 4/21/2026, and the record did not show cefpodoxime was administered after readmission. The pharmacy medication regimen review identified these irregularities, but the facility could not provide evidence that the recommendations or the hospital orders were addressed. For Resident ID #111, who was readmitted with COPD exacerbation, a pharmacy medication regimen review noted that Anoro Ellipta, a maintenance inhaler ordered on the hospital continuity of care form, had not been entered and requested clarification because the resident was on no maintenance inhalers and had worsening shortness of breath. The record did not show the recommendation was addressed when first identified in January 2026 or when repeated in February 2026. The Director of Nursing was unable to provide evidence that the recommendation had been addressed until after the issue was brought to the facility’s attention by the surveyor, and the Physician Assistant stated she was unaware of the pharmacy reviews and would have ordered the inhaler beginning in January if notified. For Resident ID #70, who was readmitted after hospitalization with ascites, the hospital continuity of care form documented furosemide 20 mg daily, but the resident continued receiving 40 mg daily from 5/6 through 5/11/2026. The pharmacy review specifically noted the discrepancy and indicated agreement to change the dose to 20 mg, yet the record did not show the order was corrected until after surveyor involvement. For Resident ID #73, with COPD and congestive heart failure, the MAR showed multiple missed doses of Vascepa and tiotropium bromide-olodaterol, and the record did not show the provider was notified of the missed administrations. The Nurse Practitioner stated she was unaware the medications were not being given as ordered, and the DNS could not provide evidence that the resident was kept free from significant medication errors.
